The greater occipital nerve originates from the second cervical spinal nerve (C0-C1). From there, it must weave its way through several tight layers of muscle, fascia, and connective tissue at the base of the skull before reaching your scalp.

When a whiplash force occurs, several distinct mechanical breakdowns can compromise this delicate pathway:

1. Direct Nerve Traction and Micro-Trauma

The violent, whip-like motion subjects the nerve to sudden stretching and compression. This direct traction can create microscopic damage within the nerve fibers and disturb normal electrical signaling, leading to spontaneous burning or shooting pain.

2. Suboccipital Muscle Spasms and Guarding

Whiplash routinely triggers severe muscle guarding. The small, deep suboccipital muscles at the base of the skull—as well as the surrounding neck and shoulder muscles—often contract chronologically to stabilize the injured spine. When these muscles remain tight, they compress the occipital nerves passing directly through them.

3. Upper Cervical Joint DysfunctionThe extreme forces of whiplash impact the joints connecting the skull to the first cervical vertebra (Co) and the first to the second cervical vertebra (C1-C2). Irritation or misalignment in these upper cervical joints triggers localized inflammation, alters spinal mechanics, and irritates the adjacent nerve roots that feed into the occipital system.

4. Scar Tissue and Entrapment

As soft tissues heal from a whiplash injury, the body lays down dense scar tissue and thickened fascia. Because the occipital nerve navigates tight anatomical tunnels, post-traumatic adhesions narrow these spaces, essentially trapping the nerve and subjecting it to friction every time you move your head.

5. Central and Peripheral Sensitisation
